State health officials imposed the largest vaccination fraud penalty in agency history after the nurse practitioner falsified records for 162 children.

A Suffolk County nurse practitioner has been ordered to pay a $544,000 civil penalty after state health officials determined she falsified vaccination records for 162 children.

The penalty represents the largest ever imposed for vaccination fraud in the agency's 125-year history, according to officials. The nurse was previously ordered to forfeit more than $1.2 million in connection with the same conduct.

The falsified vaccine cards were part of a scheme in which the nurse sold fraudulent COVID-19 vaccination documentation. Authorities said the nurse generated approximately $1.5 million through the sale of fake vaccine cards.

The civil penalty was issued by state health officials following their investigation into the falsification of vaccination records. The combined financial penalties—the new $544,000 civil fine plus the previous $1.2 million forfeiture order—total more than $1.7 million.
